The Semtech (Sierra Wireless) 5-in-1 is a low profile flat panel antenna with 4x4 MIMO cellular for 5G and 4G plus an active GNSS element. It is IP67 rated, has four SMA cellular leads and one SMA GPS lead on 10 ft cables, and mounts with adhesive using the included foam pad.
